Akutagawa had been on a mission, but someone had hit him with something, intentionally, making him pass out. The Mafia had found him, and put him in his room, waiting for him to wake up. But it wasn't good when Chuuya was nearly mad about the whole thing. When Akutagawa wakes up, he feels weird and not himself. when he checked himself in the mirror, he realized that he was now a woman or girl. (same thing) What would happen next?

I'll try to keep the reader more gender-neutral, but there'll probably be stuff that more points to the reader being a girl.
